Job summary

Caring-Hospice-Services is seeking a Registered Nurse licensed in New York to provide compassionate hospice care with a focus on pain and symptom management. You will assess patients, review histories, and support caregivers, while prioritizing urgent needs and coordinating with physicians.

The role requires independent work and strong documentation, with travel to patients' homes as part of the on-the-road duties.

Qualifications

• Licensed as a Registered Nurse in the State of New York.
• Hospice experience preferred.
• Knowledge of Medicare and Medicaid requirements outlined in licensure.
• Must possess strong patient assessment, patient management and documentation skills.
• Excellent organizational, interpersonal, telephone, documentation and communication skills

Responsibilities

Job description

The RN provides nursing care with special emphasis on control of pain and other symptoms to the hospice patient.

Job Functions for Registered Nurse:

Assess the physical and psychological status of the patient.

Reviews past and present medical history of the patient. Perceptive in planning and problem solving.

Provides emotional support to caregivers.

Responds to calls promptly when received from answering service. Is able to prioritize.

Able to work independently and make critical decisions based on available patient information.

Performs ongoing assessment of patients' conditions, physical needs and needs of family for support in giving care.

Maintains ongoing communication with prescribing physician for changes in medication and treatments as needed.

Documents hospice patient's condition and nursing interventions on clinical record.

Qualifications:

Licensed as a Registered Nurse in the State of New York

Hospice experience preferred

Knowledge of Medicare and Medicaid requirements outlined in the laws of licensure in the state(s) which he/she practices.

Must possess strong patient assessment, patient management and documentation skills.
