Job summary

Caring Hospice is seeking a Registered Nurse to provide comprehensive nursing care in a hospice setting with emphasis on pain and symptom control. The role involves frequent on-site patient assessments, family support, and coordination with physicians.

The RN should possess NY licensure, hospice experience is preferred, and strong documentation and communication skills are essential. This is a full-time position with on-the-road responsibilities to serve patients in the community.

Qualifications

• Licensed as a Registered Nurse in the State of New York.
• Hospice experience preferred.
• Knowledge of Medicare and Medicaid requirements outlined in licensure laws.
• Must possess strong patient assessment, patient management and documentation skills.
• Excellent organizational, interpersonal, telephone, documentation and communication skills.

Responsibilities

• Assess the physical and psychological status of the patient.
• Review past and present medical history of the patient; plan and problem solve.
• Provide emotional support to caregivers.
• Respond to calls promptly and prioritize.
• Work independently and make critical decisions based on patient information.
• Perform ongoing assessment of patients' conditions and family needs.
• Maintain communication with prescribing physician for changes in meds/treatments.
• Document patient condition and nursing interventions on the clinical record.

Job description

• Documents hospice patient's condition and nursing interventions on clinical record.
