La Clínica de La Raza is seeking a Primary Care provider to join an outpatient team delivering patient-centered care in a diverse Bay Area community. The position supports UCSF NP students rotating with La Clínica and emphasizes mentorship, quality initiatives, and collaborative practice.

You will work within a culturally diverse setting, triaging patients, managing acute and chronic conditions, and coordinating care with a cross-functional team across multiple sites.

Competenze

• Experience with electronic health record systems (NextGen or Epic).
• Strong interpersonal skills for staff and patients.
• Familiarity with Electronic Panel Management.
• English/Spanish proficiency preferred but not required.

Mansioni

• Provide medical triage and care management for a patient panel.
• Make referrals to specialty providers and ensure follow-up.
• Deliver evidence-based primary care with emphasis on health outcomes.
• Prescribe medications and order diagnostic tests as appropriate.
• Consult with physicians on complex cases and participate in team discussions.
