Camden Family Health is seeking a compassionate Nurse Practitioner or Physician Assistant to join a School-Based Health Center serving Webster County and surrounding communities within an FQHC.

The role emphasizes preventive and acute primary care for patients of all ages with a collaborative, team-based approach. It offers a consistent school-based schedule with no nights or weekends.

Qualifications

• Graduate of an accredited NP or PA program.
• Current and active licensure in the state of West Virginia (WV).
• DEA registration (if applicable).
• Experience in pediatrics, family medicine, or community health preferred.
• Strong clinical assessment and decision-making skills.
• Ability to work independently in school-based and outpatient/community settings.

Responsibilities

• Provide comprehensive primary care services.
• Conduct physical exams, acute care visits, and well-child checks.
• Diagnose and manage acute and chronic conditions within scope of practice.
• Provide immunizations and preventive care per guidelines.
• Coordinate care with families, school staff, and interdisciplinary care teams.
• Document all patient encounters accurately in the EHR.
• Support care coordination, referrals, and continuity of care across settings.
• Participate in health education and community wellness initiatives.
• Ensure compliance with all FQHC, state, and federal clinical standards.

We are looking for compassionate, dedicated, and experienced healthcare professionals to join our team!

We are seeking a dedicated and compassionate Nurse Practitioner or Physician Assistant to join our School-Based Health Center serving Webster County within a Federally Qualified Health Center (FQHC). While this position is based in a school setting, it also serves the broader community population, expanding access to primary care services beyond the school environment. The ideal candidate is patient-centered, flexible, and committed to delivering high-quality preventive and acute care. Webster Springs Elementary School, Webster County, Glade Elementary School, Hacker Valley Elementary School

Schedule

• School-based clinic setting with community outreach responsibilities
• 8 hour workdays M-F (aligned with school hours when school is in session)
• No nights or weekends
• Scheduling may adjust during school breaks to support community clinic coverage or outreach needs

Key Responsibilities

• Provide comprehensive primary care services
