Job summary

UNC Health in Goldsboro, NC seeks an experienced Advanced Practice Provider (NP/PA) to support care transitions in the Wayne region. The APP focuses on post-discharge follow-up and ensuring timely continuity across care settings to improve outcomes and reduce readmissions.

Coverage for overflow acute care visits is available; position is onsite in Goldsboro. The role requires NP/PA program graduation, NC licensure (or eligibility), and a minimum of two years of clinical experience.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Current North Carolina licensure (or eligible) as an NP or PA.
• Minimum of 2 years of clinical experience.

Responsibilities

• Provide post-discharge follow-up care for patients transitioning from hospital to outpatient settings.
• Perform assessments including history, physical examination, and medication reconciliation; develop and implement individualized care plans with physicians and care teams.
• Coordinate care with primary care providers, specialists, and community resources; educate patients and families on treatment plans, medications, and self-care strategies.
• Manage overflow same-day or acute care visits as needed.
• Support initiatives to improve care transitions, patient outcomes, and reduce hospital readmissions.
