Job summary

NPHire is seeking NPs and PAs to join a Sacramento-area pain management practice expanding into skilled nursing facilities. You will manage pain panels for geriatric residents, follow a fixed weekly SNF panel, and collaborate on medication management, including controlled substances, with an MD on site.

Qualifications

• Active, unrestricted California NP or PA license.
• AGACNP-BC, AGPCNP-BC, FNP-BC, or PA-C certification.
• Active DEA registration, or able to obtain one before start.
• Comfortable managing 40 to 50 visits per day.
• Fully onsite; able to travel between Sacramento area facilities.
• New grads considered with RN background in ortho, pain management, SNF, or high-volume ER.
• TEBRA or PointClickCare experience a plus | Medicare credentialing strongly preferred

Responsibilities

• See 40 to 50 residents per day in focused pain management follow-ups
• Follow an established SNF panel weekly and refine treatment plans for acute and chronic pain
• Prescribe and adjust pain medications, including controlled substances
• Work alongside physical therapy, acupuncture, and interventional services
• Guide patients and families on pain management strategies
• Keep documentation current in TEBRA and PointClickCare
• Partner closely with SNF leadership and nursing staff

Job description

A pain management practice expanding across Sacramento-area skilled nursing facilities is looking for NPs and PAs who are comfortable with a fast-paced pain panel. You'll follow the same geriatric residents weekly, manage their pain medications, including controlled substances, and work with an MD on site. Providers with pain management, ortho, physiatry, SNF rounding, or hospice experience are the priority. New graduates are considered with an RN background in ortho, pain management, SNF, or high-volume ER. Malpractice with tail coverage is included, and new seats open as facilities come on board.

You're paid per visit: $10 for each completed visit, and never less than $150 for any facility you see patients at in a day. Visits are short, focused follow-ups of about 10 minutes. Most providers see 40 to 50 patients daily, or roughly $400 to $500 a day. On days split across smaller facilities, the minimum can push that higher.

• TEBRA or PointClickCare experience a plus | Medicare credentialing strongly preferred

Schedule & Compensation

• $10 per completed visit | ~$400–$500/day at typical volume
• $150 minimum guaranteed per facility per day (a floor, not an extra payment)
• Full-time | ~32 hours/week | Schedule varies by facility
• Onsite at Sacramento area SNFs | MD on site
• Malpractice with tail coverage | Formal onboarding
• Licensure costs are the candidate's responsibility; no sign-on or relocation
