Essentia Health in Duluth, MN seeks a Palliative Care APRN to join an interdisciplinary team. You will focus on reducing symptoms and improving quality of life while collaborating with physicians and other health care professionals across care settings.
Responsibilities include inpatient palliative care with potential expansion to outpatient work. The role emphasizes patient- and family-centered care, with a shift schedule that supports a 0.4 FTE commitment and strong customer service skills.
Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention. They may care for patients ranging in age from newborn to elderly as noted in age-related category and area of expertise.
The Palliative Care APP collaborates with an interdisciplinary team to care for patients with serious illness. The focus of this care is reducing the symptoms of stress of the illness with a goal of improving quality of life for the patient and their caregivers. This patient-centered approach supports the physical, mental, emotional, and existential effects of suffering with a serious illness and helps to ensure that recommended treatments align with the patient’s goals.
Current team consists of 4 MDs, 4 Nurse Practitioners and a dedicated palliative care Social Worker and Chaplain. Position requires high level of customer service skills to establish and enhance relationships with patients, co-workers, and others. Currently, duties include inpatient palliative care with possibility of outpatient work in the future.
Shifts: Rotating Shifts: Days. Primarily Monday – Friday with possibility of weekend coverage in the future. Estimated Patients Seen Per Day: 5 - 7 Patients in a consultant role
Experience in Palliative Medicine strongly preferred. Minimum of 1-year post-degree clinical APRN experience Must have ability to work independently.
Current licensure as a registered nurse in appropriate state Current licensure as a Nurse Practitioner in appropriate state Current certification in CPR National certification in applicable area Successful completion of Essentia Health credentialing process prior to practice Building A - Duluth Clinic - 1st Street
