Impact Health PC in Glastonbury, CT, is hiring a Nurse Practitioner or Physician Assistant for flexible, physician-led opportunities in skilled nursing facilities across the Northeast.

The role offers a Monday–Friday schedule with no nights, weekends, or holidays, plus a competitive compensation package with bonuses based on performance. You will work with supportive clinicians and have access to physician-level consults when needed.

Qualifications

• Active NP or PA license in good standing.
• Minimum 2 years post-certification experience.
• Experience in SNFs, hospitalist, ED, or ICU preferred.
• Proficient with EHRs and clinical technology.
• Strong communicator with a collaborative mindset.

Responsibilities

• Deliver person-centered care using evidence-based practices.
• Collaborate with interdisciplinary facility teams.
• Develop treatment plans and manage follow-up care.
• Educate patients, families, and staff.
• Complete timely documentation (within 24 hours).
• Ensure compliance with state regulations in SNF settings.
