Crossroads Health is seeking an Advanced Practice Provider (PA/NP/APRN) to deliver comprehensive primary care within an integrated health team in Mentor, Willoughby and Euclid. The APP will provide patient-centered care in collaboration with physicians, addressing behavioral health, medication management, and preventive services.

Requirements include a Master’s degree and Ohio licensure as an APP, with experience in primary care.

Qualifications

• Master's degree required.
• Licensed in the State of Ohio as a Physician Assistant, Nurse Practitioner, or Advanced Practice Registered Nurse.
• Experience in a hospital or family practice setting as a primary care APP (1 year preferred).
• Controlled Substance Registration License in Ohio.
• Professional Liability Insurance (employer provided).
• Standard Care Arrangement or Supervision Agreement (employer provided).
• BLS/First Aid (employer provided).

Responsabilités

• Provide general comprehensive primary medical care services to patients across the life span in the office setting, including behavioral health assessment, medication evaluation and management, and prescription of medication per Ohio regulations using a trauma-informed approach.
• Provide diagnosis, screening, education, treatment, referrals, procedures, and follow-up of services.
• Coordinate ongoing treatment with transfer or discharge planning of health center patients in various settings.

Come and be a part of our Primary Care Health Center with locations in Mentor, Willoughby and Euclid. The Advanced Practice Provider (APP) is a Physician Assistant (PA), Nurse Practitioner (NP), or Advance Practice Registered Nurse (APRN). The APP works in accordance with a standard collaborative care arrangement with a physician or to provide primary care services collaboratively within a multidisciplinary health team. The APP is responsible for providing comprehensive patient-centered care in an integrated healthcare setting. This position is responsible for individual services to patients assigned by Crossroads Health in the Integrated Healthcare Center

Additionally, the Prescriber will:

• Provide general comprehensive primary medical care services to individual patients across the life span in the office setting including behavioral health assessment, medication evaluation and medication management, and prescription of medication in compliance with Ohio state regulations using a trauma informed approach.
• Provide diagnosis, screening, education, treatment, referrals, general medical procedures, and follow-up of services.
• Provide medical coordination of ongoing treatment involved with the transfer or discharge planning of health center patients in various settings.

Position Requirements:

• Master's Degree (minimum)
• Licensed in the State of Ohio as a Physician Assistant, Nurse Practitioner, Advanced Practice Registered Nurse
• Experience in a hospital or family practice setting as a primary care APP (one year preferred)
• Controlled Substance Registration License in Ohio
• Professional Liability Insurance. (employer provided)
• Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable) (employer provided)
• BLS/First Aid (employer provided)
