Job summary

Hueman is actively hiring Nurse Practitioners to complete in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members in and around Stamford, CT. This role offers a flexible schedule and travel reimbursement, with comprehensive training and dedicated scheduling support.

NPs will perform non-invasive assessments, review medical history, and ensure accurate documentation. A state NP license, national certification, and BLS are required; 12 months of paid clinical NP experience is

Qualifications

• Requires NP licensure and current national board certification.
• Master’s degree in nursing or equivalent.
• BLS certification and active NP licensure required.
• 2+? years of clinical NP experience preferred.

Responsibilities

• Travel to and from patients' homes for HRAs.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety.
• Provide basic patient education on wellness and preventative healthcare.
