American Family Care is seeking an Advanced Practice Provider (APP) to deliver high-quality care at a franchise location in California. You will work within the scope of training, collaborating with other providers to ensure efficient patient flow and high patient satisfaction.

The role requires board-certified PA or NP licensure, ability to manage diverse clinical scenarios, and flexibility to work evenings, weekends, or holidays as needed.

Qualifications

• Board certified by ANCC, AANP, or NCCPA Board Certified (ER/UC experience preferred).
• Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice; DEA number and state-controlled substance certificate as required.
• Able to multi-task and work independently; communicate effectively with staff, patients, and families.

Responsibilities

• Take complete histories, review records, develop medical assessments, and order labs/imaging as appropriate.
• Formulate diagnoses and refer patients to facilities or resources as needed.
• Institute emergency measures and stabilize patients in critical situations.
• Interpret data to guide patient management and care decisions.
• Educate patients and families about health care and promotion.
• Collaborate with staff to ensure efficient patient flow and high-quality care.
• Perform duties as assigned with flexibility for evenings, weekends or holidays.

Job Summary

The Advanced Practice Provider (APP) cares for our patients within the scope of training and approved the agreement, as outlined by their governing board. Responsibilities

• Perform complete, detailed, and accurate health histories, review patient records, develop comprehensive medical assessments, and order laboratory, radiological and diagnostic studies appropriate for complaint, race, sex, and physical condition of the patient
• Formulate medical and nursing diagnoses and institute therapy or referrals of patients to the appropriate health care facilities, agencies, and other resources of the community or physician
• Institute emergency measures and emergency treatment or appropriate stabilization measures in situations such as cardiac arrest, shock, hemorrhage, convulsions, poisoning, and allergic reactions
• Interpret and analyze patient data to determine patient status, patient management and treatment
• Provide instructions and guidance regarding health care and health care promotion to patients/family/ significant others
• Other duties and responsibilities as assigned
• Work collaboratively with staff to ensure efficient patient flow and high-quality care
• Perform other duties as assigned
• Variable schedule, including evenings, weekends, or holidays as needed

Qualifications

• Excellent communicator with staff, patients, and family
• Professional appearance and attitude
• Board certified by ANCC, AANP, or NCCPA Board Certified – (with 1 yr ER/UC experience preferred)
• Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate, as required
• Able to multi-task and work independently
• Ability to perform emergency procedures and interpret clinical data
• DOT Certification Preferred
