A leading healthcare organization in New York seeks a Registered Nurse to join their interdisciplinary care team. The role involves coordinating health and social services for older residents, ensuring they receive necessary support to age in place. Responsibilities include conducting assessments, advocating for residents, and connecting them with healthcare providers. Works as a member of an interdisciplinary care team at a NORC (Naturally Occurring Retirement Community) or Senior Living Retirement Community location. Responsible for coordinating a broad range of health and social services to support older residents aging in their own homes. Facilitates and integrates community resources and organizes additional services to enable older adults to remain in their community. Locations available throughout NYC. Works under general direction.
Licenses and Certifications: License and current registration as a Registered Professional Nurse in New York State required. CPR/BLS Certification required.
Education: Associate's Degree in Nursing required; Bachelor's Degree preferred.
Work Experience: Minimum of two years of experience as an RN in LHCSA or CHHA required. Prior experience with aging populations and clinical assessments required.
USD $40.95 - USD $51.19 /Hr.
