Job summary

Hackensack Meridian Health is seeking a Registered Nurse Non-Acute to provide nursing care within a collaborative interdisciplinary team. The role emphasizes evidence-based practice, patient-centered care, and coordination across units to achieve optimal outcomes.

The RN will triage calls, guide medications, and assist with insurance authorizations while maintaining timely documentation and communication with patients and families.

Qualifications

• BSN in progress and/or willing to acquire within three (3) years of hire.
• The ability to learn quickly and adapt to changing patient needs, a strong sense of accountability for improving the lives of our patients and their caregivers, an exceptional focus on teamwork, dedication to ongoing education and the ability and passion to deliver the highest quality of care based on a strong sense of patient focus are all required.

Responsibilities

• Provides direct nursing care following evidence-based policies and standards.
• Coordinates patient care with interdisciplinary team including physicians and other caregivers.
• Triages calls, provides medication guidance and handles insurance authorizations/denials.
• Performs comprehensive nursing assessments, planning, implementation and evaluation of care.
• Documents assessments, plans-of-care, interventions and outcomes in the patient record.
