Job summary

Hackensack Meridian Health is seeking a Registered Nurse Non-Acute to join our care team in New Jersey. The role involves direct patient care, coordination with an interdisciplinary team, and adherence to evidence-based nursing practices to ensure safe, high-quality outcomes.

Responsibilities include assessment, planning, implementation, and evaluation of patient care, with a focus on education, documentation, and multi-disciplinary collaboration.

Qualifications

• BSN in progress and/or willing to acquire within three (3) years of hire.
• The ability to learn quickly and adapt to changing patient needs, accountability for improving lives, teamwork, ongoing education, and delivering high-quality patient care.

Responsibilities

• Collects comprehensive data pertinent to the patient's health or the situation.
• Performs age and disease specific assessment / data collection independently focusing on physiologic, psychological and cognitive status.
• Recognizes clinical and diagnostic status changes in patient status.
• Communicates, coordinates, and implements the plan of care.
• Provides ongoing communication with patient / family and health care team regarding status and plans.
• Performs nursing care and treatments reflecting patient needs and evidence-based practice.
