Job summary

PACS is seeking a licensed LPN/LVN to provide direct nursing care and supervise nursing assistants in a long-term care facility in California. The role requires leadership, adherence to nursing policies, and coordination of resident care plans and discharge planning.

Qualifications

• Must possess a Nursing Degree from an accredited college or university, or be a graduate of an approved LPN/LVN program.
• Current, unencumbered license to practice as an LPN/LVN in this state (California).
• Ability to read, write, speak, and understand English.

Responsibilities

• Provide direct nursing care to residents and supervise day-to-day nursing activities by nursing assistants.
• Direct and supervise the nursing staff, ensuring adherence to facility policies and regulatory guidelines.
• Coordinate care plans, discharge planning, and implementation of nursing service procedures.
• Maintain documentation, charting, and reports for admissions, transfers, and discharges.
• Administer medications as ordered, verify resident identity, and manage narcotic records.
• Participate in quality assurance activities and collaborate with other departments for resident care.
