Job summary

PACS is seeking a qualified LPN/LVN to provide direct nursing care and supervise nursing assistants in a long-term care setting. You will ensure adherence to current standards and help maintain the highest quality of care for residents.

Responsibilities include supervising staff, keeping policies up to date, coordinating shifts, and performing essential charting and medication administration. Excellent teamwork and adherence to regulations are essential.

Qualifications

• Must possess Nursing Degree from an accredited program.
• Current active license to practice as an LPN/LVN in the state.
• Ability to read, write, and communicate effectively in English.

Responsibilities

• Direct the day-to-day nursing activities performed by CNAs and supervise nursing staff.
• Ensure compliance with facility policies, procedures, and regulatory requirements.
• Maintain Nursing Service Procedures Manual and keep it current.
• Plan shifts, services, programs, and activities with staff and support personnel.
• Complete required charting, reports, and documentation for residents.
• Administer medications as ordered, verify resident identity, and manage narcotic records.
• Coordinate with other departments; participate in QA and facility surveys as needed.
• Participate in care planning, discharge planning, and resident assessments.
