Job summary

Emerald Nursing & Rehab Legacy Pointe is seeking an No Fill Licensed Practical Nurse to provide compassionate care in a SNF setting. The position requires LPN licensure in NE or multi-state, with PRN shifts and a minimum of one shift per week.

You will administer medications, monitor residents, and collaborate with a multidisciplinary team to ensure optimal outcomes for our residents. A flexible schedule and ongoing support are provided.

Qualifications

• LPN or LVN license in NE or multi-state per state rules.
• Graduation from an accredited LPN/LVN program.
• Previous experience in skilled nursing facility or similar setting preferred.
• Current CPR/BLS certification required.
• Excellent verbal and written communication and ability to work with diverse teams.

Responsibilities

• Provide direct nursing care to residents including medications, wound care, and treatments as prescribed.
• Monitor vital signs, document progress, report changes to Charge Nurse or DON.
• Assist in development and implementation of individualized care plans.
• Maintain accurate documentation of patient care and communicate with residents, families, and healthcare team.
• Adhere to facility policies, infection control, safety protocols.
• Provide emotional support and education to residents and families.
• Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated care.

Responsibilities

• Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
• Monitoring and Assessment: Monitor residents' vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
• Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
• Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
• Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
• Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
• Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Qualifications

• Education: Graduation from an accredited LPN/LVN program.
• Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.
