Responsibilities

• Coordinate NJ DDD services and supports for individuals.
• Monitor service delivery aligned with individual needs and plans.
• Maintain timely, accurate client records and documentation.
• Communicate with families, guardians, and providers regularly.
• Collaborate with Support Coordinators and healthcare professionals.
• Support person-centered planning and progress toward goals.
