Job summary

RCM Healthcare Services seeks a Nights Hospice Field Case Manager (RN) in the Bronx, NY. The role combines home visits and facility rounds, with on-call and weekend rotation.

Qualifications

Responsibilities

Nights Hospice Field Case Manager (RN)

Salary Range: $115,000 – $125,000 per year

Location: Bronx, NY (Montefiore) Onsite / Field-Based

Position Summary

As a Nights Hospice Field Case Manager (RN), you will provide compassionate, patient-centered care to hospice and palliative patients across community and facility settings. This role involves performing comprehensive physical and psychosocial assessments, developing individualized care plans, and supporting patients and families through end-of-life care. You will collaborate with interdisciplinary teams to ensure high-quality, culturally sensitive care while promoting comfort, dignity, and quality of life.

Responsibilities

Schedule / Shift

• Shift details: Evenings/Nights (5:00pm – 1:00am or 1:00am – 9:00am)
• Hours per week: 40

Key Duties

• Conduct physical and psychosocial assessments during hospice and palliative care visits
• Develop and implement individualized nursing plans of care based on patient needs

Requirements
