Job summary

RCM Health Care Services is seeking a Hospice Field Case Manager (RN) to assess patients in home and facility settings in Bronx or Manhattan. The role requires nursing experience, NY RN licensure, and the ability to coordinate care with physicians and care teams.

Compensation includes a salary of $115,000–$125,000 plus a $15,000 sign-on bonus and excellent benefits. Training is provided for RNs with strong bedside experience.

Qualifications

• Two years of Community or Oncology nursing experience.
• Working knowledge of Word and Excel.
• Bilingual abilities a plus.

Responsibilities

• Visit hospice and palliative care patients in community and facility settings to perform physical and psychosocial assessments, and document visits electronically.
• Develop, implement, and update individualized nursing care plans based on patient needs.
• Coordinate care changes with physicians by requesting written orders when needed.
• Supervise, train, and evaluate home health aides and homemakers supporting hospice patients.
• Follow the required schedule, including one weekend per month and backup on-call coverage 2–3 times monthly, with car and escort service provided for on-call shifts.

Position Summary

When days cannot be added to a life, we add life to remaining days. Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities and hospitals.

Training Provided for RNs with strong clinical bedside experience!

Responsibilities
