Job summary

Mount Sinai Morningside is seeking a Patient Transfer Center Coordinator for the HSO Patient Transfer Center (MSH-150 East 42nd Street). The full-time night shift (7PM–7AM) coordinates inter-hospital transfers, ensuring clinical, financial, and regulatory criteria are met across Mount Sinai Health System.

The role involves coordinating referrals, registering patients, and communicating with physicians, insurers, and families to expedite approvals.

Qualifications

• Four year high school diploma or equivalent. Successful completion of training program as an Emergency Medical Technician, EMT Paramedic, and other protocol update programs as currently required. Associates Degree preferred.
• Minimum of 3 years acute care experience
• NYS Paramedic Certification, BCLS, ACLS and PALS required

Responsibilities

• Serves as the point of contact for referrals and obtains necessary information to initiate the patient’s transfer into or between Mount Sinai Health System hospitals.
• Receives transfer requests from physicians, affiliate representatives or families with demographic, insurance and clinical data.
• Responsible for complete and accurate registration of all patients in a timely manner.
• Coordinates and secures inpatient referrals to appropriate medical services as needed for the patient population.
• Connects with accepting Mount Sinai Physician to determine clinical appropriateness for transfer.
• Performs follow-up communications to finance, insurers, physicians and care team to expedite transfer approval.
• Maintains daily, weekly and monthly statistics for Administration reporting.
• Advocates for patients, families and referring physicians to promote satisfaction with access to Mount Sinai services.
• Documents all patient encounters and enters data into referral tracking system.
