Job summary

The Crestmoor in Nashville is seeking a Wellness Nurse to administer medications, supervise care teams, and deliver person-centered services in a senior community.

Candidates must hold a valid LVN/LPN license in the state and BLS/CPR certification, with 6 months to 1 year in assisted living or long-term care. Strong communication and knowledge of HIPAA are required, with weekend and holiday availability.

Qualifications

• Current LVN/LPN license in the state of employment.
• BLS/CPR certification required.
• Experience in Assisted Living or Long-Term Care preferred.

Responsibilities

• Administer medications safely and accurately per physician orders and regulatory standards.
• Oversee medication aides and conduct monthly medication reconciliation and audits.
• Coordinate daily staff assignments to ensure consistent resident care.
• Communicate with physicians, families, and care teams regarding resident needs.
• Provide direct care including ADLs and mobility support.
• Respond to medical emergencies with clinical judgment.
• Complete clinical documentation including assessments and service plans.
• Document incidents and communicate with leadership.
• Monitor care team documentation for accuracy.
• Support orientation and ongoing training of care staff.
• Assist with scheduling in collaboration with leadership.
• Attend required in-services and continuing education.
• Maintain professional appearance and hygiene, and promote respectful communication.
• Perform other duties to support resident care and operational excellence.
