Job summary

Qualifications

• Initial and ongoing health status assessment for patients.
• Document planned nursing interventions to meet patient needs.
• Collaborate with multidisciplinary team to revise care plans.

Responsibilities

• Assessment/Diagnosis - Performs initial, ongoing, and functional health status assessment as applicable to the population and or individual (30%).
• Outcomes Identification/Planning - Documents planned nursing interventions to achieve outcomes (30%).
• Implementation/Evaluation - Evaluates response to interventions and revises care plans (20%).
• Leadership - Participates in process improvement for quality and safety (10%).
• Operations - Performs additional duties as assigned (10%).
