Qualifications

• Current New York State Pharmacy License is required.
• Strong written and verbal communication skills essential for collaboration with health care professionals.

Responsibilities

• Evaluates medication orders against clinically appropriate, patient-specific parameters and documents all interventions.
• Initiates substitution of formulary medications when clinically appropriate.
• Documents all clinical activities.
• Reviews orders for IV contrast as provided by Radiology.
• Ensures proper labeling, storage, and charging for medications.
• Dispenses a 24-hour supply of unit-dose medications for new orders.
