Job summary

Crouse Health’s Pharmacy team seeks a Staff Pharmacist to join a full-time daytime/evening/night schedule at our Syracuse, NY campus. Responsibilities include evaluating orders, resolving issues with providers, and ensuring proper labeling, storage, and charging for dispensed medications.

You will use hospital and pharmacy computer systems, participate in formulary substitutions when appropriate, and maintain stock. Strong communication with health care teams is essential.

Qualifications

• Current New York State Pharmacy License is required.
• Excellent written and verbal communication with health care teams.
• Strong interpersonal skills and teamwork in a hospital setting.

Responsibilities

• Completely evaluates all medication orders against clinically appropriate, patient-specific parameters.
• Resolves any identified issues utilizing hospital guidelines or with the appropriate provider.
• Initiates substitution of formulary medications for non-formulary medications when clinically appropriate.
• Documents all interventions.
• Reviews all orders for IV contrast as provided by Radiology.
• Efficiently utilizes both hospital-based and pharmacy-based computer systems.
• Ensures that all products are appropriately labeled prior to dispensing.
• Participates in maintaining a working stock of medications and supplies.
• Ensures proper storage of medications.
• Ensures that a patient, nursing unit or department is charged for all medications dispensed/ materials utilized.
• Dispenses a 24 hour supply of unit of use medications to individual patients for all new orders.
• Checks unit dose carts filled by pharmacy assistants. Checks prepackaged unit dose medications prepared by pharmacy assistants.
• Written and oral communication and interpersonal skills to deal effectively with other health care professionals and hospital employees.
