Job summary

UnityPoint Health is seeking a Transfer Center Intake Nurse to serve as the first point of contact for providers seeking patient admission. This role coordinates provider communication, triages transfer requests, facilitates patient placement, and supports transfers to higher levels of care when needed.

Qualifications

• Completion of an accredited nursing program and NCLEX exam.
• 2+ years of hospital RN experience in ER, ICU, or intermediate care.
• Active RN license in state of practice or Compact/Multi-State license; BLS certification (on hire).

Responsibilities

• Perform nursing triage to identify providers, clinicians, and staff needed for medical conversations and transfers.
• Assess available information to ensure accurate patient placement, and know when to request more data.
• Ensure compliance with EMTALA, HIPAA, CMS, and other regulatory requirements.
• Adhere to policies and workflows on patient transfers, placement, and capacity.
