Job summary

Qualifications

Responsibilities

• Administers medications using two identifiers and documents promptly.
• Develops and updates individualized care plans and assessments.
• Coordinates care with interdisciplinary team and communicates changes promptly.
• Ensures accurate documentation and hand-offs between shifts.
