Qualifications
- Licensed practical nurse (LPN) or registered nurse (RN) license in the state.
- At least one year of experience in a long-term care setting preferred.
- Knowledge of MatrixCare software preferred.
Responsibilities
- Provide direct patient care and monitor residents' conditions.
- Document vital signs and care activities accurately.
- Administer medications and monitor responses.
- Collaborate with healthcare team to plan and evaluate care.
- Participate in discharge planning and patient education.
