Qualifications

Responsibilities

• Facilitates identification and resolution of gaps in care from hospitalization, at discharge, and into ambulatory care.
• Leads the development and implementation of complex plans of care with defined goals in collaboration with the interprofessional team, patient, family, or caregiver.
• Analyses practice gaps and provides feedback to enhance safe patient care and satisfaction.
