Qualifications

Responsibilities

• Communicates with multidisciplinary team members and provides patient data.
• Establishes individualized nursing diagnosis and formulates care plan.
• Identifies and addresses patient/family care needs promptly.
• Initiates discharge planning to meet individual patient needs.
• Provides education regarding hospitalization and discharge instructions.
