Job summary

Qualifications

Responsibilities

• Delivers safe and appropriate nursing care per the nursing skills inventory and patient needs.
• Coordinates planning for delivery of nursing care with an interdisciplinary team.
• Collaborates with the healthcare team to achieve outcomes based on data and standards of care.
• Coordinates discharge and transfer plans; ensures patient education and documentation.
