Job summary

Gulfside Healthcare Services is seeking an RN for field hospice care in the Clearwater area. The RN will manage patient and family needs in private residences, assisted living facilities, and SNFs, providing assessment, intervention, and education to maximize comfort and quality of life.

Responsibilities include supervising LPNs/CNAs, case management, and ensuring compliance with professional standards. Requires Florida RN license and prior hospice/home health experience preferred.

Qualifications

• Current Florida RN license is required.
• Experience in hospital, home health or hospice preferred.
• Ability to supervise LPNs and CNAs in field settings.
• Strong ethics and professional demeanor.
• Excellent time management, critical thinking and judgment.

Responsibilities

• Case manages patient/family needs in the field.
• Performs ongoing assessment of physical, psychological, social and spiritual needs; initiates intervention.
• Oversees supervision of LPNs, CNAs, and assigned staff.
• Maintains timely documentation and adheres to policies.
• Educates patients and families about hospice care.
• Communicates with Clinical Care Manager regarding patient status and risk management.
