Job summary

Qualifications

• In lieu of RN experience, residency completion or LPN experience may be accepted.

Responsibilities

• Conduct nursing assessment and develop an individualized plan of care with patient/family.
• Coordinate and communicate plan changes with the healthcare team.
• Deliver direct patient care per established nursing plan.
• Ensure patient safety and adherence to policy, documentation and standards.
• Participate in discharge planning to ensure continuity of care.
