Job summary

Qualifications

• Perform the Nursing Process including assessment, diagnosis, outcomes, planning, implementation and evaluation with evidence-based practice.
• Provide relationship-based, patient-centered care across diverse populations.
• Partner with patients and families using bedside shift reports and interdisciplinary rounds.
• Practice according to current clinical practice standards.

Responsibilities

• Coordinate delivery and documentation of safe, quality patient care with interdisciplinary communication.
• Promote teamwork and collaboration while supporting fiscal responsibility and staff retention.
• Engage in performance improvement and research activities to advance nursing practice.
• Plan for care across the care continuum and develop protocols in collaboration with care managers.
