Qualifications

Responsibilities

• Assess, plan, implement, and evaluate care using a multidisciplinary approach.
• Deliver medications, monitor patient status, and document care provided.
• Support activities of daily living and patient education as appropriate.
• Coordinate with care teams for transfers, discharges, and referrals.
• Ensure high-quality outcomes while adhering to standards and safety.
