Job summary

Qualifications

• Active RN license in Massachusetts.
• Experience delivering direct patient care using the Nursing Process.
• Ability to coordinate care across the discharge planning continuum.

Responsibilities

• Utilizes the Nursing Process for delivery and management of patient care from admission to discharge.
• Performs patient assessment/reassessment according to standard procedure.
• Develops and coordinates individual plan of care including discharge planning.
• Directs, supervises, coordinates and evaluates nursing care within the assigned setting.
• Delegates appropriately to others in accordance with job description and competency.
