Job summary

The Registered Nurse at Rochester Regional Health’s Canton-Potsdam Hospital provides comprehensive assessment, care planning and direct delivery of patient care in a patient- and family-centered model. The role requires a NY RN license and BLS, with BSN and certification preferred.

Responsibilities include full nursing care, developing a plan of care with families, patient education, discharge planning and supervising unlicensed staff. This is a 12-hour night shift with weekend requirements.

Qualifications

• Graduate of an accredited nursing school program.
• Current, unrestricted New York State RN license; AHA BLS on hire.
• BSN and national certification preferred.

Responsibilities

• Performs a comprehensive patient assessment to identify patient and family needs.
• Develops and implements a comprehensive plan of care involving the patient and the family.
• Performs and documents direct patient and family centered care and education.
• Identifies and responds to patient conditions to achieve positive outcomes.
• Performs discharge planning to ensure safe transition of care.
• Directs and delegates care provided by unlicensed team members.
• Provides world class customer service to patients and families.
