Responsibilities

• Collect and analyze assessment data, identify expected outcomes and develop a plan of care with patient/family and other interdisciplinary team members.
• Identify interventions to attain outcomes, implement plan, evaluate progress, and document process.
• Promote effective functioning of the health care team and delegate tasks per NC Nurse Practice Act.
