Responsibilities

• Assist House Supervisor / Patient Placement Coordinators with Capacity Management workflows.
• Assist with coordination of patient transfers; collect condition and diagnosis information, obtain face-sheet, and collect medical records/images.
• Serve as a liaison to provide real-time bed status updates to referring facilities.
• Communicate status updates with transferring facilities and update House Supervisors on clinical changes.
• Coordinate transportation for transfers (ED and inpatient).
• Assist with Out-of-Network transfer workflows.
• Facilitate Provider to Provider communications.
• Assist House Supervisor with staffing assignments.
• Expedite patient flow through prioritization of resources (EVS, Facilities, etc).
• Coordinate delivery of stat medical record requests.
