Job summary

Qualifications

• Master’s degree in social work, counseling or related field.
• LCSW/LPC/LSW licenses issued by Pennsylvania.
• Ability to remain patient and calm in stressful situations.
• Knowledge of community resources and effective referrals.
• Understanding psychosocial/medical needs to form integrated treatment plans.

Responsibilities

• Oversees utilization management: authorization, denial and appeal processes.
• Ensures discharge planning documentation is clear, timely and aligned with treatment.
• Conducts clinical assessments for interpretation and treatment with patients and families.
• Provides case management and care coordination for clients.
• Intercedes to help patients achieve community stability.
• Liaises between patients, referrals and funders.
• Manages voluntary and involuntary commitment processes per state law.
• Participates in department quality improvement activities.
