Job summary

Central City Concern is seeking an On-Call Patient Navigator for the Recuperative Care Program in Portland, OR. The role involves crisis management and intensive case coordination within a multi-disciplinary team, supporting patients through the RCP stay and linking them to medical and housing services.

The position is on-call with evening and weekend shifts (swing/graveyard) at Evergreen Crossing, offering a competitive hourly rate.

Qualifications

• At least one year of experience in a pre-hospital or post-hospital setting (medical case management, social work, counseling, behavioral health, EMT, etc.).
• CPR certification required prior to start date.
• Bachelor's degree in a healthcare-related field preferred.
• Knowledge of homelessness causes, medical/psychosocial sequelae, and housing systems.
• Ability to work in fast-paced environments and with diverse populations.

Responsibilities

• Fill in for regular Recuperative Care staff as needed based on program capacity and task needs.
• Coordinate acute and ongoing medical care for patients in Recuperative Care in collaboration with staff.
• Serve as primary contact for care coordination and resource linkage for caseload patients.
• Assist patients with prescribed medications and support self-administration plans.
• Perform welfare checks and charting for participants in the Recuperative Care Program.
• Maintain and update patient charts in the EMR system; ensure timely documentation.
