Job summary

Qualifications

• Basic Life Support Certification (BLS) for healthcare providers.

Responsibilities

• Assess physical, social, and psychological status and coordinate care with interdisciplinary team.
• Develop individualized plan of care and involve patient, family, and team in education and referrals.
• Direct the care team, delegate tasks, and manage time, supplies, and resources.
• Evaluate patient response to interventions and revise the plan as needed.
