Job summary

Qualifications

Responsibilities

• Assess, develop, implement, and evaluate care plans with a multidisciplinary team.
• Conduct patient assessments across biophysical, psychosocial, and discharge planning needs.
• Administer medications, IVs, and treatments per physician orders.
• Respond to life‑threatening situations per protocol and manage emergencies.
• Document care accurately in the electronic medical record and ensure safety.
