Job summary

Qualifications

• Current, unencumbered LVN license in California.
• Current CPR certification.
• SNF experience preferred.
• Ability to lead CNAs, multitask, and make sound clinical decisions.
• Strong communication, documentation, and assessment skills.
• Knowledge of PCC preferred.

Responsibilities

• Provide direct nursing care in accordance with resident care plans and clinical standards.
• Supervise and guide nursing assistants during the assigned shift.
• Admit, transfer, and discharge residents as needed.
• Document nursing notes, physician orders, treatments, and resident status.
• Prepare and administer medications and treatments per physician orders.
• Respond promptly to changes in residents' conditions, notifying supervisors and physicians.
• Ensure compliance with facility policies, infection control practices, and safety procedures.
• Participate in ongoing quality assurance efforts and facility surveys.
• Maintain a professional, supportive environment for residents, families, and staff.
