Job summary

Regency Integrated Health Services in Austin seeks a Licensed Vocational Nurse (LVN) Charge Nurse for Heritage Park Rehabilitation and Skilled Nursing Center on the 10pm-6am shift. This full-time role supports high-quality resident care.

You will observe and document resident changes, administer medications, work with physicians, supervise aides, and ensure care plans are followed, with focus on safety and dignity.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.
• Active Texas LVN license in good standing.
• Experience working in a skilled nursing facility is preferred.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with shifts.
• Oversee aides and ensure incident reporting per policy.
• Discharge residents per facility policy.
• Communicate with resident's point of contact about status or changes.
