Qualifikationen

• Must hold a current LPN or RN license.
• Minimum of 1 year of experience working with elders or disabled individuals.
• Demonstrated leadership capabilities.
• Knowledge of Assisted Living industry and Dementia care or Skilled Nursing preferred.

Aufgaben

• Monitor the health and wellbeing of residents and support wellness with leadership.
• Administer medications and communicate with resident support services and families.
• Maintain clinical quality assurance and reflect the PSP during wellness visits.
• Lead and support all elements of wellness and oversee changes in care plans as needed.
• Assist Wellness Director with completing monthly wellness visits and PSP updates.
