Qualifications

• Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
• Associate degree in practical nursing is preferred.
• Currently holds an unencumbered LPN license with licensure privileges.
• Continuing education and CEU requirements are expected.

Responsibilities

• Provide professional nursing care for residents in long-term care under supervision of a nurse, advanced practice provider, or physician.
• Administer medications or treatments as ordered and ensure accuracy of care plans.
• Maintain documentation of resident care, outcomes, and communications with health care providers.
• Collaborate with an interdisciplinary care team to support resident well-being.
