Job summary

Lifeatheritage is seeking a Full-Time 3rd shift Licensed Practical Nurse (LPN) / Registered Nurse (RN) in Huntington, Indiana. The role involves direct nursing care, medication administration, and collaboration with a multidisciplinary team to support long-term resident wellbeing.

On-site, every other weekend and holiday coverage is required. Applicants must hold current LPN and RN licenses in good standing.

Qualifications

• RN and LPN licenses required in good standing with state board of nursing.
• Experience in long-term care, skilled nursing, or geriatric care preferred.
• Excellent communication and documentation abilities.
• Ability to work well in a team and maintain professionalism.

Responsibilities

• Provide direct nursing care to residents, including administering medications and treatments as prescribed.
• Monitor residents for changes in condition and report findings promptly.
• Assist in developing, implementing, and evaluating individualized care plans.
• Document all care, observations, and resident interactions accurately and timely.
• Ensure compliance with infection control and safety policies.
• Collaborate with interdisciplinary team for coordinated resident care.
• Supervise and support CNAs in daily care delivery.
• Communicate effectively with residents, families, and staff.
• Participate in shift reports and ongoing education.
