Job summary

Interim HealthCare of Augusta is seeking a Private Duty LPN or RN for full-time night shifts (7pm–7am) to care for a pediatric patient in Jonesboro, TN. Great compensation and very flexible scheduling, with 50 hours weekly and self-scheduled days.

Responsibilities include providing home-based care under RN supervision, collaborating with a care team, assessing patients, and educating families on plans of care and home safety. Current TN LPN/RN license and CPR required.

Qualifications

• Graduate of an accredited practical nursing program and active RN or LPN license in TN.
• CPR certification is required.
• Knowledge of state and federal home health regulations.
• Clinical nursing proficiency and compassionate patient care.

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases—often after hospital discharge—under the supervision of an RN
• Work with a team of physicians, RNs, CNAs and aides to deliver quality patient care
• Take patient vitals, do assessments, document progress and report changes to RN supervisor
• Assist RN in carrying out patient’s plan of care and teaching patient self-care techniques
• Educate family on the patient’s plan of care, medication and home safety
• Collaborate with interdisciplinary team to coordinate patient care and plan for discharge
