Job summary

Rehabilitation Hospital of Oak Creek is seeking a Licensed Practical Nurse for the Night shift in a brand-new 40-bed inpatient rehabilitation facility. You will assess patients, initiate nursing interventions, educate patients and families, and collaborate with an interdisciplinary team to optimize function and quality of life.

The role requires an Associate's Degree and an active Wisconsin LPN license (or compact state).

Qualifications

• Requires critical thinking and decisive judgement in a fast-paced rehab setting.
• Ability to work with minimal supervision in a stressful environment.

Responsibilities

• Performs patient assessments and identifies needs.
• Initiates nursing interventions as appropriate.
• Provides patient and family teaching during rehab stay and before discharge.
• Provides care appropriate to patient condition and age.
• Collaborates with the interdisciplinary team.
• Participates in CQPI.
• Documents medical necessity in the chart.
• Completes and revises the Interdisciplinary Care Plan.
